Employers seeking verification of valid registration may do so free of charge by accessing the LPN Public Registry at http://www.clpnbc.org/lpn_list.php or by submitting a completed verification request form (see below). Please be advised that a nominal fee of $2.00 plus GST per name requested will be charged.
Verification lists are available only after January 1.
Step 1: Provide employer name, contact and email address in space provided
Step 2: Complete columns A through C. Provide SIN# if available. ONLY LPN employees should appear on list.
Step 3: Cost is $ 2.00 plus GST for every name included on your list (including duplicates or Non LPN's). Invoices will be issued upon completion of verification.
Step 4: Email completed list to info@clpnbc.org, subject line: Verification list.
Payment is due within 30 days. Verification list and Invoice will be sent by email to contact provided.
Only verification requests received in template provided by CLPNBC will be completed. Verification lists will be completed within 10 days of receipt)
